Third Party Records Request

Cariend is an independent custodial records provider, a company chartered with storing records for closed medical facilities. We’ve been retained by certain providers to store their patient records and provide release of information. Cariend is not a directory or search resource for records. Please do not contact us for records unless you have been specifically notified that we have your records.

The process you are about to follow is required by HIPAA law. It is our utmost priority to keep patient information secure and provide records in a HIPAA-compliant manner. Cariend reserves the right to charge third parties for copies of records, in accordance with applicable laws.

Please do not send personal health information (PHI) such as social security numbers, date of birth, or photo IDs via email.  To request a copy of a patient’s medical records, you must follow the process outlined below. You will first provide basic information by completing the required fields below. Once you’ve submitted this information, within 24 hours you will receive an individual email with a link to upload your designated authorization request form and/or court order for the records. Please do not email this information for security purposes, rather wait for the secure link to be emailed to you.  Please do not attempt to fax your forms, we do not have an active fax number. The secure link is the only means for providing the forms. If you do not see an email with the upload link within 24 hours of submitting your contact information, please check your email SPAM folder. If you do not follow the process outlined below, it only slows our response capabilities and further delays the processing of your request so please read through the steps carefully.

  1. Complete the fields below providing your basic contact information, patient and provider information. Cariend will then verify if we have custody of the records for that particular provider and/or patient.

    • Receive a secure upload link via the email you provided in the contact fields above. Please do not send anything until you receive the link.
    • Upload the designated authorization request form and/or court order for the records to the link provided in the email. We do not have a fax number, so you will not be able to fax the request to us. Uploading the designated authorization via the secure link emailed to you is the only way to process the request.
